By attending this event, I hereby grant to NEW TRIBE CHURCH, its officers, representatives, agents, attorneys, employees and assigns (individually and collectively, “New Tribe”) the right and permission to record, film, photograph, videotape, and/or live-stream my participation and appearance, to exhibit, reproduce, distribute or otherwise exploit same, in whole or in part, without restrictions or limitation, for any and all purposes, whether commercial or promotional, through any and all mediums or media, whether now known or hereafter devised, and to use my name, likeness, photograph, image, voice and/or biographical material in connection therewith, without consideration, compensation, or reporting to me. I hereby waive any and all rights or interests of any kind, nature or description that I might have with respect to any of the foregoing, including, without limitation, any and all copyright or copyright-related claims, trademark, or claims of unfair competition. I, on behalf of myself, my family, heirs, and personal representatives, hereby and forever expressly release, discharge and agree to indemnify and hold harmless New Tribe from and against any and all claims, demands and causes of action which I or any third party might have arising from or relating to any of the above. I HAVE READ AND UNDERSTOOD THE FOREGOING AND HEREBY WARRANT THAT I AM 18 YEARS OF AGE OR
OLDER AND FULLY UNDERSTAND THE CONTENTS OF THESE TERMS & CONDITIONS. IF I AM UNDER THE AGE OF 18 YEARS, I HEREBY WARRANT AND REPRESENT THAT MY PARENT OR LEGAL GUARDIAN HAS READ, UNDERSTOOD, AND SIGNED THESE TERMS & CONDITIONS ON MY BEHALF, WHICH SIGNATURE SHALL BIND SAID PARENT OR LEGAL GUARDIAN TO ALL OF THAT TO WHICH I HAVE REPRESENTED AND AGREED, ABOVE.
